RIO Ready: Student Request - Special Consideration Form
This is an application for special consideration, that may be submitted to the institution to take unexpected or extenuating circumstances into consideration during the marking of an assessment.
Fred (Student) had a low assessment mark/grade due to a health condition which prevented him from giving full focus on the assessment. Fred goes into the Student Community and applies for a special consideration to be sent to the institution for further action. The information captured from this form are:

| Examination or Assignment Title | The description of the exam/assignment/assessment. |
| Description | More details on the special consideration e.g. why do you need special considerations etc. |
| File Upload | Enables student to upload supporting documents. |
